INSTRUCTIONS

Output form to your printer; we will only accept forms via the U.S. Post Office, the cancellation date is used to prioritize your registration. You can either fill the form out then print, or print then fill the form out by hand.

NOTE: If you fill the form out on-line and change screens before printing, the form will be cleared.

  • Send one form per camper including paid or free children. Copy or find additional forms available for printing at www.contradancers.com/heaven.
  • Enclose check payable to Almost Heaven Committe for all campers whose forms are enclosed.

Scholarships: A limited number of work scholarships are available. Please indicate on the registration form if you are interested. Your camp fees will be lowered based on the number of hours you work. Usual jobs include work in dining hall or sweeping dance floors. Full payment is expected at time of registration. You will receive reimbursment of a portion of your camper fees if you receive a scholarship.
